Emergency services were called to the Myall Way intersection at Tea Gardens about 4.50pm after a white Suzuki Jimmy and a Hino rigid tabletop collided.
Police confirmed the Suzuki's driver, a 90-year-old man, died at the scene.
The 24-year-old truck' driver was not injured.
He underwent a roadside breath test that returned a negative result.
He was subsequently taken to Newcastle's Mater hospital for mandatory blood and urine testing.
His vehicle had been transporting two steers at the time.
Both animals escaped injury in the crash.
Port Stephens-Hunter police are investigating and closed a southbound lane of the highway as well as a right-hand turning lane for northbound traffic into Myall Way.
