Police say the crash happened at 3am after a silver Holden Commodore lost control going round the Beach St roundabout and then crashed into a boat.
The driver, a 32-year-old Forster male was cut out of the car and went to hospital.
Two of four passengers were also hospitalised, with the rear seat passenger getting flown to Sydney from Port Macquarie Base Hospital.
Manning-Great Lakes Local Area Command duty officer Allan Fidock said they were still investigating the cause of the crash.
“Investigations have commenced which will include all possible factors,” Mr Fidock said.
“At this stage it is too early to tell what caused the crash, but speed and weather may be a factor.”
