Lake came back from two goals down to win 3-2 on penalties and take the title.
Kaine Morgan was the hero for Lake, heading in a brilliant goal with virtually the last touch of the game.
The full time whistle went seconds after the ball landed in the back of the net.
Neither side was able to score in golden goal extra time, so the match went to penalties.
Saints keeper Jacob Lyttle and his opposite number Dean Bensch produced a couple of brilliant saves in an exciting shootout.
Lake coach Tony Russ said the win capped off a brilliant season for the minor premier Lake team.
Saints finished second in the regular season.
Russ was pleased with how his team dug itself out of a big hole to win.
"The boys hung in there," he said.
It was the latest thrilling match between the two teams. Lake's minor premiership came on goal difference.
"We've had a win each against each other, and a draw," he said.
"There's nothing between the two teams, so it was a good effort from them too."
